Residents / Advanced Practice Providers

Prior to working at a site, the following Orientation Modules must be completed annually by residents and advanced practice providers. After completing all modules and you understand the information presented; complete, sign, and submit to your residency / advanced practice provider program coordinator or heath care facility coordinator the ‘Confidentiality Agreement and Acknowledgement of Orientation Modules‘ form (Reviewed 05/14/2026). Signing this form verifies you completed and understand all modules, in addition to abiding by the confidentiality requirements.
