Prior to working at a site, the following Orientation Modules must be completed annually by residents and advanced practice providers. After completing all four modules and you understand the information presented; complete, sign, and submit to your residency / advanced practice provider program coordinator or heath care facility coordinator the ‘Confidentiality Agreement and Acknowledgement of Orientation Modules‘ form. Signing this form verifies you completed and understand all four modules, in addition to abiding by the confidentiality requirements.
